Mono County is hosting a Vaccine Clinic for students aged 12 years old and older. This includes all students and their family members. The vaccination clinic will be held on Tuesday, May 18th, and Wednesday, May 19th, from 10 am to 2 pm at the Mammoth High School Gym. Parents have the option of taking students to the clinic for the vaccine or school staff can escort students with a signed consent form to and from the clinic back to MHS.
Appointments are not necessary to participate in the clinic. Students will need to bring a signed consent form to receive the vaccine. The parent consent forms are attached and in the main office for students coming to the clinic independently. I have attached the consent forms in Spanish and English. All students will receive a hard copy of the consent form to take home tomorrow. Students will need to bring the signed consent form and turn it into the MHS main office on the day of the vaccines by 8 AM. In addition to the consent form, I have attached a parent letter from Mono County Public Health Department and a fact sheet regarding the vaccine for you to review.
The second vaccination shot will be scheduled for administration on June 7th and June 8th. More information will be shared regarding this clinic at the end of May. If you have any questions please don't hesitate to contact the main office.
